Can chronic Hepatitis B ever be completely cured, or is treatment always lifelong?A functional cure — defined as hepatitis B surface antigen (HBsAg) loss with or without development of anti-HBs antibodies — occurs spontaneously in only about 1% of patients per year on treatment. This is very rare and represents an exceptional response. Elimination of covalently closed circular DNA (cccDNA) from the liver — the true molecular reservoir of HBV — is not achievable with current antivirals. However, this field is intensely active in research, with multiple approaches including novel capsid assembly modulators, RNA interference therapies, and immunotherapeutic approaches in clinical trials.
